HUNTINGTON, WV (WOWK) – Marshall Health is supplying free sporting activities physicals by appointment to center and higher university college students on Thursday, June 2. The physicals will be performed by the Marshall Sports Drugs Institute at 2211 Third Avenue in Huntington from 5-8 p.m.
All present documentation is expected for appointments, including camp varieties, proof of insurance coverage, and any other existing health care file. If the participant does not see a Marshall Wellness service provider for most important care, they must also convey immunization documents for the college student.
Participants have to convey a West Virginia Secondary School Activities Fee bodily sort, or Ohio High University Athletic Affiliation physical. The sorts must be filled out and signed by a guardian or lawful guardian. Mothers and fathers can schedule an appointment as a result of their web site.
